Your loyalty program just got better!

Connect Rewards…

… is a loyalty program for members with combined hospital and extras cover at GMHBA. It generates Connect Rewards dollars which you can spend to enhance your benefits.

To recap, a single GMHBA membership with combined hospital and extras cover generates $50 after the first year of membership, increasing to $75 a year up to a maximum balance of $500. Connect Reward dollars are doubled for all family, couple and single parent memberships that have combined hospital and extras cover with GMHBA. These memberships receive $100 after the first year of membership and then $150 per year thereafter up to a maximum balance of $1,000. Until now, your Connect Rewards dollars could be spent on reimbursing a hospital excess or increasing benefits or annual limits for selected services.

Connect Rewards Plus!

Based on feedback from members, we’re proud to unveil Connect Rewards Plus! This enhanced loyalty program features all the advantages of the previous program – PLUS AN OPTION TO REDUCE THE MEDICAL GAP! Under the new program you can now spend your Connect Rewards dollars on in-patient medical gaps, which may significantly reduce or eliminate your out-of-pocket expenses on in-patient services.The new ‘gap’ Connect Rewards Plus benefit is effective for hospital admissions from the 1st of January 2007.

If you have a hospital admission which results in out of pocket medical expenses, we’ll write to you within 90 days of your hospital discharge to ask if you would like to use your Connect Rewards Plus dollars towards the cost of the medical gap.  In this letter, we’ll include your current Connect Rewards Plus balance.  In order to spend Connect Rewards dollars on in-patient medical gap, your Connect Rewards Plus balance and out of pocket medical expenses must be at least $50 at the time of discharge from hospital.

Hospital only members

If you have hospital cover only*, you’re not currently participating in this loyalty program. Now, there’s never been a better time to consider adding extras cover to your membership. Aside from the fact that you’ll be covered for a range of services not covered by Medicare such as dental, physiotherapy and optical, you’ll become eligible for our enhanced loyalty program – Connect Rewards Plus!

So visit a branch or call our Customer Service Centre on 1300 4 GMHBA (46422) to add extras cover to your membership from around $4 a week# for singles $8 per week# for families. If you add extras before 31st December 2006, we’ll waive the normal 2 month waiting period for physiotherapy, occupational therapy, pharmacy and general dental (conditions apply).

Extras Cover only members

If you have extras cover only*, you’re also not currently participating in this loyalty program. Now, there’s never been a better time to consider adding hospital cover to your membership. Aside from the fact that you’ll be covered in a private hospital or as a private patient in a public hospital, you’ll also become eligible for our enhanced loyalty program – Connect Rewards Plus!

So visit your local branch or call our Customer Service Centre on 1300 4 GMHBA (46422) to add hospital cover to your membership. If you add hospital cover before 31st December 2006, we’ll waive the waiting period for hospital services that normally attract a 2 month waiting period (conditions apply).

How much do I get?

The Connect Rewards Plus benefit entitlement added to your membership is:

• Single membership: $50 after first completed year, $75 per year in subsequent years;

• Families/couples/single parent memberships: $100 after first completed year, $150 per year in subsequent years.

To keep earning Connect Rewards Plus dollars, you must be up to date with your payments. The maximum you can accrue is $1,000 for families/couples/single parent memberships and $500 for single memberships.

What else can I claim with my Connect Rewards Plus dollars?

Reduce your Excess
You can reduce the hospital Excess payable when you are admitted to hospital (if applicable).

Increase major dental benefits
You can increase your benefits for crowns, bridgework, dentures, surgical extractions, implants, inlays/onlays, gold fillings or orthodontic treatment and reduce any out-of-pocket costs.

Increase limits
You can increase your annual limits for Pharmacy, Physiotherapy and chiropractic/osteopathic services provided these services are already included in your extras cover.

Save money on services not covered
You can use your Connect Rewards Plus to claim up to 70% of the cost of the following when purchased from a GMHBA approved provider:

 

Remedial Massage

Up to $17 per visit

Swimming lessons

70% of the cost (when recommended in writing by a doctor for the purpose of preventing or improving a specific health condition/s)

Blood Pressure Monitor

70% of the cost (limited to 1 monitor per membership every 3 years upon written recommendation by doctor)

Annual membership to Diabetes Australia

70% of the cost

Annual membership to Asthma Association

70% of the cost

Joint supports

70% of the cost (upon written recommendation by doctor)

First aid courses

70% of the cost

Quit smoking programs

70% of the cost

Lift for Life diabetes program

70% of the cost

Orthopaedic shoes

70% of the cost

Melanoma surveillance photography

70% of the cost (limited to one claim per person per membership lifetime)

Nicotine replacement therapy patches

70% of the cost (upon written recommendation by doctor)

Please note:

• GMHBA does not recommend or endorse any health or medical program, therapy or appliance in respect of which Connect Rewards Plus benefits are offered or paid. Some programs, treatments or appliances should not be undertaken or used without medical advice.

• Services listed within the program must be provided by practitioners who are registered with recognised bodies approved by GMHBA. Contact us on 1300 4 GMHBA (46422) to confirm if a supplier is recognised.

• In circumstances where family/couples/single parents memberships (with Connect Rewards Plus accumulated in excess of the single membership limit) changes to a single membership, the existing membership may retain the Connect Rewards Plus accrued. However, you’ll not accrue any further Connect Rewards Plus until your accruals return to within the single membership limit of benefit entitlements.

• Connect Rewards Plus is a membership reward. Connect Rewards Plus entitlements cannot be transferred from one membership to another. For more information about GMHBA’s Connect Rewards Plus call 1300 4 GMHBA (46422).
